Palm Coast Office Directions

Take Exit 284 off of Interstate 95, travel West on State Road 100 for 1/2 mile.
Turn right on Town Center Blvd., then take the 2nd right onto Hospital Drive.
Town Center Medical is the large green building on your left.
Enter thru the south tower our office will be shared with Dr. Zak – Town Center Gastroenterologist in suite 250. on the 2nd floor.
Taking the elevator the suite is straight ahead when you exit the elevator; Taking the stairs: turn left at the top.
