New patients are always welcome!

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. 

Everyone at our office is at your service.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours of notice. This courtesy on your part will make it possible to give your appointment to another patient.

Health Insurance Plans

Our office participates with most health insurance plans, which cover certain procedures.  Covered procedures will vary with each insurance company and our office staff will assist you in determining eligibility.  Please contact our office at (386) 677-9044 to find out if we accept your insurance.  

Patients are responsible for deductibles, co-payments, and remaining balances.  Patients also have the option of paying office and surgical fees in advance before receiving their insurance reimbursement check directly from the insurance company.

Financial Policy

Co-payments and co-insurances are due at the time services are rendered.  Forms of payment accepted by the office are check, cash, insurance assignment and most major credit cards.
